The Science Behind Emsella
BTL Emsella is a novel technology for urinary incontinence patients. It provides completely non-invasive electromagnetic stimulation of pelvic floor musculature for the purpose of rehabilitation of weak pelvic muscles, and restoration of neuromuscular control to treat urinary incontinence.
This is performed by High Intensity Focused Electromagnetic (HIFEM) technology which has been proven safe for both men and women.
As our body ages, incontinence problems can occur due to the weakening of the pelvic floor muscles (PFM) supporting the bladder as a result of prostate surgery, childbirth or menopause. When the PFM weaken, pelvic organs like the bladder is not supported and the patient can experience involuntary urine loss, which is incontinence.

HIFEM Technology
HIFEM technology triggers intense PFM contractions by targeting neuromuscular tissues and inducing electric currents. Electric currents depolarize neurons resulting in concentric contractions, lifting up the PFM evenly. Key effectiveness is achieved by targeting electromagnetic energy at the entire pelvic floor area, then inducing in-depth penetration, and stimulating supramaximal contractions at the pelvic floor area. This induces a more efficient growth of myofibrils, resulting in muscle fibre hypertrophy as well as the creation of new protein strands and muscle fibres. HIFEM technology modifies the PFM by creating deep PFM stimulation and restoring neuromuscular control.

Supramaximal Contractions
Maximal voluntary contraction (MVC) is the greatest amount of tension that can be developed and held physiologically by our muscles when we exercise. Usually this is done only for a split second. Muscle contraction occurs in the brain when we exercise or move which sends signals to the motor neurons to innervate the PFM fibres.
Contractions with a tension higher than MVC are defined as supramaximal. HIFEM technology is able to create supramaximal PFM contractions and hold them for a couple of seconds. The key to the effectiveness of HIFEM technology is in the gradually increasing intensity of the focused electromagnetic fields and frequency of pulses, which result in unsurpassed vigorousness of the contractions. In each session, about 11,200 supramaximal contractions are performed on the PFM while sitting on Emsella’s throne. This method is extremely important to patients with urinary incontinence who need to remodel their PFM, but are not able to perform this high-repetition rate pattern or Kegel exercises due to PFM weakness.

During HIFEM Treatment
HIFEM therapy protocol takes around 30 minutes and consists of 3 different phases. These phases create an intense awakening of the PFM by stimulation and relaxation of the PFM. Repetition of these phases results in adaptation, modulation and strengthening of the PFM.
